First Appointment

Print bring the following completed forms to your appointment.

1. Patient registration form
2. Patient’s immunization record

Please arrive about 15 minutes early for the paperwork to be entered into your chart.

Please bring your insurance card and any copay amount for each visit. If you have no insurance coverage, payment in full is expected at the time of the visit. If your card is not yet available, we will need the following information to file a claim:

1. The name of the insurance company, any network of which it may be a part, and the claim mailing address
2. The name, address, date of birth and social security number of the person who is the primary policy holder
3. The ID number of the patient and the policy holder
4. Group number
5. Employer of the policy holder with contact phone number
